On this site you have access to the research questionnaire.

Questionnaire files

Following you can download the questionnaire as pdf-file (Adobe) or as doc-file (MS Word). The files comprehends the same questions and the same structure. Questions can be answered by clicking the relevant answer box or by entering a word or a phrase into the empty space.

Both files are interactive forms; the pdf-file furthermore includes a self check among the check boxes and offers more comfort. In the case that you donnot have a specific preference, please use the pdf-file based questionnaire. Additional notes concerning the file formats can be found here.
