Additional notes to file formats
In order to read and to fill in the pdf-form you might need the Adobe Acrobat Reader. The programm is free of charge.
You can download the Adobe Reader for free on Adobe's website.
As an alternative you might use the Foxit PDF Reader, a free program which is recommended by PC journals, too.
The download file of this program is quite small, requires no system installation, hence it is ready to start within just a few clicks. In comparison to Adobe Reader this program opens pdf-files very fast. Even large and complex files will be opend very fast.

The MS Word-file works with all version from Word 97 to Word 2007. It can also be opened with Open Office.
